We are curerently looking for experienced Qualified Social Workers to work in a busy Adult Social Care team.
You will use medical, physical disability, psychological and psychiatric knowledge and experience to enable clients to live as independently as possible within the limitations of their disability; through the use of disability prevention / rehabilitation advice and techniques, home care, day care, respite care, equipment, minor and major adaptations and moving and handling risk assessments
You will need to provide a professional assessment and care management service to older people across the borough, through the efficient and effective management of a client caseload
To be accountable for the accurate and timely recording of allocated client data on My Carefirst
Accepting client cases, as allocated by their Assistant Team Manager, in line with the departmental protocols for case allocation and workload management
