You will work on behalf of a local authority and primary care trust to develop and implement a jointly agreed work plan to commission effective services within budget for Older People, Palliative Care and Physical Disability care groups ensuring that these meet the identified needs of the population of the local authority and are in line with National and local targets.

 Lead the development, production and review of three year care group commissioning strategies for the LA and the NHS that address the wider determinants of health and tackle health inequalities, national and local targets and need. Ensure that the housing needs of the nominated care group are supported through active participation in local groups such as care group accommodation sub groups.

 Support, develop and maintain positive and constructive partnerships with users, carers and statutory, voluntary and private sector providers.

 Advise the PCT Board and Council Executive on the requirements placed on the organisations arising from national policy developments, for example the Older People's National Service Framework.

 Produce plans for service change, commissioning and decommissioning of services and following approval lead any public consultation and negotiation required to implement these. Work with providers to build capacity to reshape, remodel or develop services in line with the commissioning strategies and new national targets or incentives.

 Masters degree in a relevant clinical/social care subject or management/professional equivalent gained through post-graduate experience with work experience that enables the post holder to demonstrate:

 In depth knowledge of the relevant care groups - needs, service models and national and local targets and the interface between these.

Experience in all aspects of the commissioning process - strategy development, market management, procurement, service monitoring, decommissioning and contract negotiation
